FAQ

Do I need a referral from my doctor?

If you have a private health fund or paying out of pocket, you DO NOT need a referal to see a dietitian.

If you have a DVA gold or white (condition-specific) card, you will need to get your GP to complete a D0904 – DVA request/referral form and send it to us.

If you wish to claim rebate from Medicare, you NEED to get a Chronic Disease Management (CDM) Care Plan* from your GP. You will be entitled to 5 dietitian consultations per calendar year. Patients under a CDM/ ECP plan are entitled to a Medicare rebate of $54.60 for each consultation.

*Consult your GP to ensure you meet the criteria for the care plan
