Do I need a referral from my doctor?

If you have a private health fund or paying out of pocket, you DO NOT need a referral to see a dietitian.

If you have a DVA gold or white (condition-specific) card, you will need to get your GP to complete a D0904 – DVA request/referral form and send it to us prior to your consult.

If you wish to claim rebate from Medicare, you NEED to get a Chronic Disease Management Care Plan* or Eating Disorder Plan from your GP.

Chronic Disease Management Plan will provide you with 5 subsidised dietitian consultations per year.

Eating Disorder Plan will provide you with 20 subsidised dietitian consultations per year

Patients under either plan are entitled to a Medicare rebate of $55.10 for each consultation.

*Consult your GP to ensure you meet the criteria for the care plan
